The official cause has not been determined, but it looks like a USB Type-Micro B to USB Type-C adapter may have been involved.
Today's news was even more definitive -
These cables 'remembered' the power setting from one device to another, which could lead to the wrong voltage and current frying your electrical gear, potentially causing a fire. There's a great explanation of the issue in this YouTube video, Why Anker recalled USB Type-C cables
